I would say look towards your family doctor. LRGH, for instance, has fast track, but its billed thru the ER, so insurance claims can be a nightmare. I get a referral from my doctor and it helps get me a lower rate, otherwise I get charged an ER visit, and my insurance won't cover it unless I get admitted

Now, if you have a tooth ache and your town just purchased a new ladder truck, there is a movement underfoot for free coverage, but that's another story.
