I think the only requirement is that it has reflectors on each side.

Even if not a requirement, you should. Any commercially made one does.

We never put our name on it, vaguely recall you are supposed to as well. But it was never a problem. Anything that broke lose on that portion of the lake ended up on our beach any way.
