I'm not asking for the names of these folks involved. A simple explanation of what mistakes were made is warranted. Maybe there were no mistakes and it was just a freak accident.
It's no different than when a bad car accident happens and the reports say speed, alcohol, distracted driving whatever were a factor. Is that an invasion of privacy? Not in my mind. It's sharing of information that might prevent someone else from also getting hurt in the future
